Blue Mountain Nursing Home is in a small rural area. Our home is attractive, functional and well maintained. We have a large dining room and activity area. There is also a large visiting area for family. We offer a friendly, well-trained, licensed staff, which provides a lot of personal attention for each individual resident. We have staff who are very experienced in long term care and a low staffing turn over rate.

Front Street

Our facility offers activities inside and outside the building. We are a locked facility, with a fenced courtyard and a mini-bus for transports and outings. Our building and rooms are air-conditioned and heated for comfort. Each resident is visited by his/her attending physician every 30-60 days, depending on the need of the resident.

Our facility is staffed 24 hours a day with a licensed nurse and certified nursing assistants. Delicious home cooked meals are prepared under the supervision of a certified dietary manager.

The full-time social service and activity department work to provide the best psychosocial well-being based on the individual needs of each and every resident that lives in our home.

Prairie City Church

Blue Mountain Nursing Home's activities include gardening, picnics, barbecues, fishing trips, weekly summer drives, and weekly lunches at the senior centercompare nursing homes and many more. We also have a licensed beautician on staff 4 days a week for hair and nail care.

Placement in long term care can be a difficult decision. With the assistance of the compassionate and caring staff at Blue Mountain Nursing Home we can help make this transition easier.

Our annual survey was completed in April, 2006, and we were once again deficiency free (7 years running). We are very proud of this accomplishment. The surveyors were very pleased with the care that is provided to our residents.

Blue Mountain Nursing Home would like to thank Millie Mansfield for the quilts that she makes for us to raffle off. Some of the proceeds go to the Resident Council Fund.
